Short answer: Anavar and TRT are not two versions of the same treatment. Anavar is a common name for oxandrolone, an oral anabolic steroid. In 2023 the FDA withdrew approval of oxandrolone tablets because of safety and effectiveness concerns, so no FDA-approved oxandrolone product is available in the United States today. Testosterone replacement therapy (TRT) is an FDA-approved, prescription treatment for men with diagnosed low testosterone. [1,2,3,4]
Key takeaways
- FDA approval of Oxandrin (oxandrolone) and its generic versions was withdrawn as of June 28, 2023. In September 2023 the FDA determined the drug had been withdrawn for reasons of safety or effectiveness. [1,2]
- Oxandrolone is still a Schedule III controlled substance. So is testosterone. [5,6]
- The oxandrolone label warned of peliosis hepatis (blood-filled cysts in the liver), liver tumors that were sometimes fatal, and cholesterol changes linked to atherosclerosis. [5]
- Anavar does not replace testosterone. It suppresses the body’s own testosterone production. [5,7]
Important safety information
Oxandrolone products sold today are not FDA-approved medicines. Its label warned of serious liver injury, including liver failure and internal bleeding, and of harmful cholesterol changes. It can make warfarin much stronger and cause bleeding. In women it can cause virilization, and some of those changes are permanent. Seek urgent care for yellowing skin or eyes, severe abdominal pain, or unusual bleeding. [1,5]
Is Anavar still FDA-approved?
No. The FDA withdrew approval of Oxandrin (oxandrolone) tablets and four generic oxandrolone applications as of June 28, 2023. In September 2023 it determined that oxandrolone had been withdrawn from sale for reasons of safety or effectiveness and said it will not approve generic versions. The FDA’s drug database lists every remaining oxandrolone application as discontinued. [1,2,3]
A 1984 FDA advisory committee unanimously found no evidence of efficacy for oxandrolone. Given the label’s safety warnings, the FDA judged its benefit-risk profile unfavorable and said it would not be considered safe and effective if reintroduced today. [1,2]
So there is no Anavar prescription a US clinician can write for an FDA-approved product. [1,3]
What Is Anavar (Oxandrolone)?
Anavar is the name most people use for oxandrolone, a synthetic anabolic steroid taken by mouth. The last FDA-approved US brand was Oxandrin. Like other anabolic steroids, it is a synthetic derivative of testosterone, so the answer to “is Anavar a steroid?” is yes. Oxandrolone vs Anavar is not a real comparison: they are the same drug. [5]
Origins and Medical Background
Oxandrolone was first approved in the United States in 1964. Its label described it as add-on therapy to help patients regain weight after extensive surgery, chronic infections or severe trauma, to offset protein breakdown caused by long-term corticosteroids, and to relieve bone pain from osteoporosis. That is what Anavar was prescribed for until approval was withdrawn in 2023. [1,2,5]
Key Mechanism
What does Anavar do? Oxandrolone acts like a male sex hormone. Its label stated that complete separation of anabolic (tissue-building) and androgenic (masculinizing) effects has not been achieved, so androgenic side effects can still occur. It also lowers the body’s own testosterone by blocking the pituitary signal (luteinizing hormone) that tells the testes to make it. [5]
Comparison with Testosterone
When people compare Anavar vs testosterone, the key point is that testosterone is the hormone itself. TRT replaces it in men whose bodies cannot make enough because of a diagnosed medical condition. Oxandrolone is a modified steroid that does not replace testosterone; it suppresses it. For a broader look at how TRT differs from anabolic steroid use, see our separate guide. [5,6]
In gym slang, searches such as “Anavar vs test” or “Anavar and test” use “test” to mean testosterone. Testosterone and Anavar are both controlled substances, but only testosterone has FDA-approved products and an approved medical use in men today. [1,6]
Anavar vs TRT at a glance
Testosterone vs Anavar differs on every point that matters to a patient: legal status, approved use and safety monitoring. The table compares oxandrolone vs testosterone using FDA documents and the Endocrine Society guideline. [1,6,8]
| Anavar (oxandrolone) | TRT (testosterone) | |
|---|---|---|
| US approval status | FDA approval withdrawn June 28, 2023; withdrawn for reasons of safety or effectiveness [1,2] | FDA-approved products available [4] |
| Approved use | Formerly: add-on therapy to regain weight after surgery, infection or trauma [5] | Men with low testosterone caused by a specific medical condition (primary or hypogonadotropic hypogonadism) [4,6] |
| Controlled substance | Schedule III [5] | Schedule III [6] |
| Form | Oral tablets [5] | Testosterone cypionate: intramuscular injection [6] |
| Key labeled risks | Peliosis hepatis, liver tumors, cholesterol changes, warfarin interaction, virilization in women [5] | Raised blood pressure, high red blood cell count (polycythemia), blood clots, reduced sperm production [4,6] |
| Effect on natural testosterone | Suppresses it [5,7] | Replaces it; can reduce sperm production (oligospermia at high dosages) [6] |
| Monitoring | Label called for periodic liver tests and cholesterol checks [5] | Testosterone level, hematocrit and prostate cancer risk [8] |
Claimed Benefits of Anavar vs What Studies Show
The benefits claimed online are mostly about muscle and leanness, but evidence for oxandrolone comes from people with illness-related weight loss, not from healthy athletes. Its label stated that anabolic steroids have not been shown to enhance athletic ability, and FDA advisers found no evidence of efficacy in 1984. [1,5]
Clinical Uses and Fitness Claims
In a randomized trial of 262 men with HIV-associated weight loss, oxandrolone produced dose-dependent gains in body weight and body cell mass. Weight also rose in the placebo group, and only some doses beat placebo after 12 weeks. The same trial found suppressed testosterone and gonadotropins, higher liver enzymes and LDL cholesterol, and lower HDL cholesterol. [7]
Fitness use is non-medical use of a Schedule III drug with no FDA-approved product. Claims of better endurance or recovery in healthy people are not supported by the sources reviewed here. [2,5]
Women’s Use & Special Considerations
Oxandrolone can cause virilization in women: a deeper voice, excess facial and body hair (hirsutism), acne and clitoral enlargement. The label warned that some of these changes are irreversible even after the drug is stopped. It was also contraindicated in pregnancy because it can masculinize a fetus. [5]
Online “before and after” photos do not show these risks and are not evidence of typical results. There is no safe dose or duration of oxandrolone for non-medical use.
Anavar and TRT: How They Interact
Anavar does not belong in a TRT plan. TRT and Anavar work in opposite directions on your own hormones. TRT treats diagnosed hypogonadism with an FDA-approved testosterone product. Oxandrolone has no approved product and no approved role alongside testosterone, and no evidence reviewed for this article shows that combining them is safe. [2,4,6]
Can Anavar be added to TRT?
Adding Anavar to TRT is not an established or FDA-supported treatment. The testosterone label notes that testosterone abuse usually involves doses above the approved range combined with other anabolic steroids, and that such abuse can lead to serious heart and psychiatric problems. For a man on TRT, Anavar adds the liver, cholesterol and warfarin risks of oxandrolone. [5,6]
Questions about taking Anavar on TRT, Anavar with TRT, or TRT with Anavar all have the same honest answer: there is no FDA-approved oxandrolone to add, and no labeled dose for this use. Oxandrolone TRT is not a real treatment category. [2,3]
Can a TRT clinic prescribe Anavar?
Searches for an “Anavar TRT clinic” assume a clinic can legally offer it, but a US prescriber has no FDA-approved oxandrolone product to prescribe. If a TRT clinic, Anavar seller or website offers oxandrolone, it is not offering an FDA-approved medicine. [1,2,3]
What a TRT clinic can legitimately do is evaluate symptoms of low testosterone and, if hypogonadism is confirmed, discuss testosterone therapy for diagnosed hypogonadism and its risks. [6,8]
Effects on Testosterone Production
Oxandrolone lowers natural testosterone. Its label explained that it suppresses luteinizing hormone and, at large doses, can suppress sperm production through follicle-stimulating hormone. In the HIV trial it significantly suppressed both hormones and total and free testosterone. Taking Anavar without testosterone therefore leaves the body’s own testosterone suppressed. [5,7]
TRT can also reduce sperm production; the testosterone label lists oligospermia at high dosages. The Endocrine Society advises against starting testosterone in men planning to father a child soon. If you are on TRT, Anavar does not “fix” suppression; it adds another suppressing drug. [6,8]
What is TRT and who is it for?
TRT is testosterone given to men who lack or have low testosterone because of a medical condition, such as testicular failure or pituitary disease. Its safety and effectiveness for age-related low testosterone have not been established. Using Anavar for TRT is not possible: oxandrolone does not restore testosterone. [4,6]
Diagnosis requires symptoms and signs of low testosterone plus testosterone measured in the morning on at least two separate days and found to be below normal. [6,8]
Guidelines advise against starting TRT in men with breast or prostate cancer, a prostate nodule or raised PSA without urologic evaluation, a high hematocrit, untreated severe sleep apnea, severe urinary symptoms, uncontrolled heart failure, a heart attack or stroke within the past six months, or a clotting disorder. [8]
Legal & Prescription Status
Oxandrolone and testosterone are both Schedule III controlled substances. The difference is that testosterone has FDA-approved products, while oxandrolone’s approvals were withdrawn in 2023. [1,5,6]
Is Anavar Legal in the U.S.?
Is Anavar legal? It remains a Schedule III controlled substance, but there is no FDA-approved oxandrolone product to prescribe. FDA’s notice states that distributing the withdrawn tablets in interstate commerce without an approved application is illegal and subject to regulatory action. Anything sold as Anavar online is not an FDA-approved medicine. [1,3,5]
Prescription Oversight & “Legal” Alternatives
Supplements marketed as “legal Anavar”, an “Anavar substitute” or an “oxandrolone alternative” are not the same as oxandrolone and are not an FDA-approved form of it. If you have lost muscle because of illness or have symptoms of low testosterone, a clinician can look for the cause and discuss approved treatments. [2,3]
Safety & Hormonal Balance
Is Anavar safe? The FDA concluded that oxandrolone’s benefit-risk profile is unfavorable. Its label warned of peliosis hepatis, liver tumors, cholestatic hepatitis with jaundice, harmful cholesterol changes, fluid retention, high blood calcium and prostate enlargement or cancer in older men. [2,5]
Labeled risks of oxandrolone
- Liver. Peliosis hepatis can go unnoticed until liver failure or internal bleeding occurs. Liver tumors were usually benign, but fatal malignant tumors were reported. [5]
- Cholesterol and heart. HDL (“good”) cholesterol falls and LDL cholesterol can rise, changes the label linked to atherosclerosis and coronary artery disease. [5,7]
- Fluid and heart failure. Edema, with or without heart failure, could be serious in people with heart, kidney or liver disease. [5]
- Bleeding with warfarin. Oxandrolone can greatly increase the effect of warfarin; in a label study, warfarin doses had to be cut by about 80% to 85%. [5]
The label listed these contraindications: known or suspected prostate or male breast cancer, breast cancer with high calcium in women, pregnancy, nephrosis and high blood calcium. It called for periodic liver tests, cholesterol checks and, at high doses, hemoglobin and hematocrit. [5]
Monitoring and risks during TRT
TRT is monitored on a plan: symptoms and side effects, testosterone level (aiming for the mid-normal range), hematocrit, and prostate cancer risk. [8]
Testosterone can raise blood pressure; the FDA found this with all testosterone products. Labeled risks also include polycythemia, blood clots in the legs or lungs, and reduced sperm production. In 2025 the FDA removed boxed-warning language about increased cardiovascular risk after the TRAVERSE trial found no increase in men treated for hypogonadism. [4,6]
The bottom line on Anavar vs TRT
Anavar is not a safer or milder version of TRT. Oxandrolone lost its FDA approval in 2023 over safety and effectiveness concerns, while testosterone remains an approved treatment for diagnosed hypogonadism, with known risks that need monitoring. [2,4]
Whether to start testosterone is a medical decision made after testing, not a choice between two muscle products. Advanced TRT Clinic works with licensed physicians and the Beluga Health, P.A. medical group. If you have symptoms of low testosterone, book a consultation for an evaluation with lab work.
