Diabetic Retinopathy: An Important Ozempic-Specific Concern
One side effect that deserves specific attention in Ozempic (but not in off-label weight loss prescribing) is diabetic retinopathy worsening. In the SUSTAIN-6 trial, which enrolled patients with type 2 diabetes and established cardiovascular disease, diabetic retinopathy complications occurred in 3.0% of semaglutide patients vs. 1.8% of placebo patients. This was primarily observed in patients who had pre-existing diabetic retinopathy and experienced rapid glycemic improvement.
The mechanism is the well-documented phenomenon of early worsening of diabetic retinopathy with rapid HbA1c reduction (also seen with insulin intensification). The retina, which has adapted to chronic hyperglycemia, can experience paradoxical deterioration when glucose is reduced rapidly. Patients with pre-existing diabetic retinopathy should have an ophthalmology baseline assessment before starting Ozempic and be monitored during the first year of treatment, particularly if HbA1c improvement is rapid.
Pancreatitis and Gallbladder Disease: Understanding the Risk
Pancreatitis
Acute pancreatitis has been reported with Ozempic and the GLP-1 receptor agonist class broadly. In SUSTAIN-6, the rate was low but the signal is present across multiple trials. The practical guidance is straightforward: any patient on Ozempic who develops severe upper abdominal pain, particularly if it radiates to the back and is accompanied by vomiting, should stop the medication immediately and seek emergency evaluation. Ozempic should not be restarted if pancreatitis is confirmed. Patients with a history of pancreatitis or gallstone disease represent a higher-risk population for this adverse event.
Gallbladder disease
GLP-1 receptor activation affects gallbladder motility, reducing contractility and predisposing to bile stasis and gallstone formation. In SUSTAIN-6, cholelithiasis was reported more frequently in semaglutide patients than placebo. Rapid weight loss (which independently increases gallstone risk) compounds this effect. Patients should be counseled on gallbladder disease symptoms (right upper quadrant pain after fatty meals, potentially with fever and jaundice) and report these promptly.
